Employee injuries are to be submitted using the online form at the link below and done so immediately upon notification of injury, but no later than 24 hours after an injury has occurred. Following the submission of the Supervisor's Report, the Employee Report is to be completed by the Employee ONLY to document the incident in their own words.
Once both the Supervisor's and the Employee's Reports are received by Risk Management, please contact our office at 361-695-7210 if medical treatment is requested by the Employee. If the incident requires emergency medical transportation, call 911 then notify Risk Management.
